South Thames Foundation School is a part of NHS South East Coast

All foundation doctors joining STFS are required to agree to the following:
1. I confirm that I accept a place within the South Thames Foundation School (STFS) and the Foundation Training Programme that I have been allocated to.
2. I confirm that I will have achieved a primary medical qualification as recognised by the General Medical Council (GMC) by the time I am scheduled to commence an F1 programme.
3. I understand that it will be necessary for me to obtain Full Registration with the GMC in advance of commencing an F2 programme.
4. I agree to register with Doctors.Net.UK / obtain an e-mail address. (Currently doctors.net is the default e-mail address for correspondence unless another secure and reliable address is agreed)
5. I understand that STFS will use my Doctors.net.uk e-mail address (or other agreed address) as the primary method of communication and agree to check this account at least once per week.
6. I agree to inform STFS immediately of any change to the details above.
7. I agree to check the STFS website for updates on a weekly basis.
8. I agree that STFS may make both the information above and programme allocation information available to other relevant third parties including but not limited to Deanery/Trust administrative staff/other Foundation Schools/UK Foundation Programme Office, NHS Education Scotland (e-portfolio), e-Learning for Healthcare.
9. I agree to always have at the forefront of my clinical and professional practice the principles of Good Medical Practice for the benefit of safe patient care. I agree that I should be aware that Good Medical Practice requires doctors to keep their knowledge and skills up to date throughout their working life, and to regularly take part in educational activities that maintain and further develop their competence and performance.
10. I agree to ensure that the care I give to patients is responsive to their needs, that it is equitable, respects human rights, challenges discrimination, promotes equality, and maintains the dignity of patients and carers.
11. I agree to abide by the processes and regulations established by STFS. This includes maintaining regular contact and attending meetings with supervisors, tutors and STFS staff as required by responding promptly to communications from them.
12. I agree to ensure that I develop and keep up to date my learning e-portfolio which underpins
the training process and documents my progress through the programme and supports my career planning.
13. I agree to use the training resources available optimally to develop my competences to the
standards set by the Foundation Programme Curriculum and the other requirements for satisfactory completion of F1 and the Foundation Programme.
14. I agree to participate proactively in the appraisal and assessment process and to undertake all of the assessments required to obtain an Attainment of F1 Competency Certificate/Foundation Achievement of Competency Document.
15. I agree to acknowledge that as an employee within a healthcare organisation I accept the responsibility to abide by and work effectively as an employee for that organisation; this includes participating in workplace based appraisal as well as educational appraisal and I acknowledge and agree that details of my performance/assessments/application details may be made available to the appropriate Director of Medical Education/Clinical Tutor/Foundation Training Programme Director/Educational Supervisor, the Foundation School Director and the Dean .
16. I agree that the Medical School from which I graduated and STFS may have access to the information contained within my Medical School/STFS files respectively if deemed necessary by the Medical School Dean or STFS Director.
17. I agree that the Medical School from which I graduated may have access to my Foundation assessment and demographic data for the purpose of tracking progression/monitoring outcomes.
18. I agree that my anonymised demographic data may be circulated to relevant third parties for the purpose of tracking progression/monitoring outcomes/research.
19. I agree to engage with quality management processes and any other activities that contribute to the quality improvement of training e.g. by completing the on-line GMC Trainee Survey.
