Document Requests

From time to time, it may be necessary for you to request documentation from STFS.   Common requests include:

  • Letters relating to jury service (see Jury Service for further details)
  • Confirmation of current status as a foundation doctor
  • Confirmation of completion of the foundation programme
  • Copy of Attainment of F1 Competence form (5.1)/ARCP Outcome Form
  • Copy of Foundation Achievement of Competence Document (FACD – 5.2)/ARCP Outcome Form

STFS will provide certified copies of original certificates which it holds but will not release original versions of documents.  Documents will usually be sent via e-mail as PDF documents, however, it is possible to request hard copies of documents to be sent by 1st class mail. 

STFS aims to respond to all document requests within 5 working days, however, more detailed requests may require longer.  If this is the case, we will contact you and provide you with an update regarding the timescale.
